CORI/KidSafe Form * - Massachusetts Legislation has made it mandatory that a CORI/KidSafe Form be completed for every adult volunteer who interacts with children in the NYSA league in any capacity. If you completed this form after June of 2005 and you have not changed your last name, you need not complete another form at this time.
